چکیده

Introduction: Duodenal neoplasms are relatively rare entity, most often being cases of duodenal invasion by malignant masses developed in the duodenopancreatic region. The malignant infiltration of duodenum can be made either by a primary malignant duodenal tumor, either by invasion of a neighborhood malignancies, the most common being pancreatic cancer. The differentiation between these two malignant entities is absolutely necessary for the type of treatment chosen. Materials and methods: The paper comprises a retrospective study, which includes 147 patients admitted and treated in a period of 7 years, being analyzed the images and the examinations results performed during 2009-2015, which were confronted with surgery protocols, where applicable, or endoscopic explorations and histological examination results. Results: In the study group, 25 had been diagnosed with primary duodenal adenocarcinomas and the rest were pancreatic adenocarcinomas which invaded the duodenum. In terms of radioimaging , were analyzed the localization of tumors on the duodenal segment, the presence and the characteristics of lacunas and stenoses, the dimension of duodenal wall infiltrated by the tumor, and also the modifications of bile ducts. Conclusions: If endoscopy cannot be performed, radio-imaging exams remain the only methods that can guide the starting point of a malignant tumor in duodenopancreatic region. It is important to know whether the tumor has duodenal or pancreatic origin for establish therapeutic strategy, the survival rate being different.

Semiological characters and morphopathological-radiological correlations in duodenal malignancy.

Localized duodenal neoplasms are relatively rare entities, most often representing invasions of the duodenum by malignant masses developed in the duodeno-pancreatic region. The paper makes a more exact analysis of radiological semiology of duodenal localized tumors and seeks to determine through this analysis the role of radiological examinations in the support for the anatomopathologist. The s...

Unusual presentation of a gastrointestinal stromal tumor of the duodenum mimicking an inflammatory enlargement of a peripancreatic lymph node

Gastrointestinal stromal tumors (GISTs) are low-grade malignant mesenchymal tumors of GI tract and are believed to originate from malignant transformation of the interstitial cells of Cajal from their precursors [1]. Duodenal GISTs are usually exophytic and appear as submucosal swellings [2]; however, in the absence of specifi c mucosal changes or anatomical variations of the duodenal lumen, it...

Brunner's Gland Hyperplasia: Treatment of Severe Diffuse Nodular Hyperplasia Mimicking a Malignancy on Pancreatic-Duodenal Area

Brunner's gland hyperplasia is a benign tumor of the duodenum and it is rarely associated with clinical symptoms. We report on a 64-yr-old man with Brunner's gland hyperplasia who had undergone a duodenocephalo-pancreatectomy. The reason is that he presented upper gastrointestinal obstructive symptoms and the esophagogastroduodenoscopic finding revealed the lesion to be an infiltrating type mas...

Duodenal carcinoma from a duodenal diverticulum mimicking pancreatic carcinoma.

An 81-year-old man was found to have a pancreatic head tumor on abdominal computed tomography (CT) performed during a follow-up visit for sigmoid colon cancer. The tumor had a diameter of 35mm on the CT scan and was diagnosed as pancreatic head carcinoma T3N0M0. The patient was treated with pylorus-preserving pancreaticoduodenectomy. Histopathological examination showed that the tumor had grown...

Duodenal-preserving resection of the head of the pancreas and pancreatic head resection with second-portion duodenectomy for benign lesions, low-grade malignancies, and early carcinoma involving the periampullary region.

HYPOTHESIS Duodenal-preserving resection of the head of the pancreas (DPRHP) and pancreas head resection with segmental duodenectomy (PHRSD) can be alternatives to standard pancreaticoduodenectomy for benign periampullary lesions. DESIGN Retrospective analysis of patients requiring surgery for benign and borderline malignant tumors of the periampullary region. SETTING Tertiary care referral...
